  • Most individuals and families do not have discussions about health care wishes and quality of life considerations prior to a serious illness or medical event. This can result in added stress and conflict and, ultimately, medical decision making may not reflect what you or your family member might have chosen for yourselves.

  • Discussions that occur prior to illness can provide peace-of-mind at any age or stage of life and avoid putting family members in the position of making difficult decisions on your behalf.

  • Commonly held understanding around some intensive medical interventions can be inaccurate

Advance Care Planning

Consultation Components

  • Session 1:

    A facilitated discussion of your values and wishes as they pertain to potential, future medical treatments, interventions, and proxy health care decision makers.

  • Session 2:

    Education around medical decision making and advance directives. This will include a review of publicly available resources. However, advance directive completion is deferred to your healthcare providers and, as applicable, legal aid.

  • Consultations are available either for individuals or family units. The standard consultation format entails two sessions and consults are completed via video conference.
