  • Most individuals and families do not have discussions about health care wishes and quality of life considerations prior to a serious illness or medical event. This can result in added stress, conflict and, ultimately, medical decision making that may not reflect what you or your family member might have chosen for yourselves.

  • Discussions that occur prior to an illness can provide peace-of-mind at any age or stage of life and may prevent a situation where family members have to make difficult decisions on your behalf.

  • The components and outcomes of CPR and life-sustaining medical supports are not commonly well understood by non-medical professionals.

Advance Care Planning

Consultation Components

  • Session 1:

    A facilitated discussion of your values and wishes as they pertain to potential, future medical treatments, interventions, and proxy health care decision makers.

  • Session 2:

    Education about medical decision making and advance directives. This includes a review of publicly available resources.

    *Please note that formal advance directive completion will be deferred to your healthcare providers and, as applicable, legal aid.

  • Consultations are available either for individuals or family units. The standard consultation format entails two sessions and consults are completed via video conference.
